| Request must be postmarked within 30 days of enclosed sales receipt. Please allow 2-4 weeks to verify request. One replacement per customer in a 12-month period. |

- Original UPC code from package.
- Your Full Name, Complete Return Address and the Name of our Product you would like as your replacement.
